In preparation for your first appointment, please download and fill out the following forms to bring with you.  They help us gather some basic information, and describe what to expect in counseling, as well as our general office policies.  If you are a part of a couple or family seeking counseling together, you will each need to sign separate forms.

General Forms you may need during the course of counseling are available by clicking on the links below.  Also, you can obtain a copy of the HIPAA Notice of Privacy Practices, which is required by the federal government and provides information regarding the use and disclosure of your medical information and counseling records.

If you would like your counselor to speak to another counselor, medical doctor, school official, family member or another individual regarding your care, you and your counselor will need to complete the following forms that authorize the appropriate individuals to share information with each other about your treatment.
